Teacher, Educational Assistant, Receptionist and Nurse.
Please Note: When the paperwork is finished an appointment will need to be made
to process the information and complete a criminal background check.

Substitute Application
I-9 (Verification of Employment) must present
DRIVER'S LICENSE AND SOCIAL SECURITY CARD OR
driver's license and birth certificate OR a passport
at the time of appointment
W-4 Tax form - Federal
W-4 Tax form - VT (both need to be completed
if you are residing in VT)
Confidentiality Form
Child Abuse Form (informational only)
Reimbursement Form (this form allows you
after substituting 5 times to be reimbursed for
the criminal background check. If you
are going to substitute in all the districts
the district will pay for 1 out of 2 background
checks.
Please provide 3 names with complete mailing addresses
on the application form. The reference will receive a form to complete.
Teacher Substitute - $70.00 per day for 4 hrs or more
Educational Assistant - $60.00 per day for 4 hrs or more
Nurse Substitutes - must have RN License for
corresponding state - $125.00/day

NOTE FOR TEACHING POSITIONS: In order to be considered for a teaching position with SAU 70 you must have the proper state certification or a letter from the corresponding State Department of Education confirming that an alternative plan is in place
